Delegate Information


Please enter your name in the field below as you want it to appear on your name badge and certificates of completion. For example, "Major John Doe Jr, D.Min."
Because this disaster training institute is limited to 200 people, the number of delegates from any one organization will be limited. Each organization has been given several slots, however if they do not use their reserved slots by September 21st, they will be made available to other organizations. Please make sure that you have been officially endorsed by your organization leadership to use one of the reserved delegate slots.

Lodging


Note: The $150 registration fee covers four nights of lodging from the dates of Monday, October 26 through Thursday, October 29th. The rooms nights you select will be booked for you at the hotel through your registration form. However, if you wish to stay additional nights, you will need to book these on your own, and at your own expense, at the hotel website which has a link on the front page of the institute website.

For Tuesday and Wednesday, either choose one two-day workshop for Tuesday/Wednesday or one workshop for Tuesday and another one for Wednesday.
Important note to participants of the two-day ServSafe® Manager Training:  
  • This course requires pre-class preparation.
  • There is a nonrefundable $60 fee added to the cost of the institute registration to cover the cost of the workbook and the examination.

Registration Fee:  $150
The $150 delegate registration fee will cover; four nights of lodging (room and tax), an evening reception on Monday, breakfast, snacks and drinks during breaks, lunch Tuesday through Thursday, dinner on Wednesday, and a brunch on Friday.
