REQUIRED DOCUMENTS:
You MUST bring:
- a picture ID
- proof of residency
- Social Security Cards for everyone in the household**
- proof of all income
** We cannot process your application without a Social Security Card for every person listed. Failure to produce proper documentation will result in your application being denied.
